Why does a transfer from my bank fail?

Why does a transfer from my bank fail? It seems to be set up correctly, but when actioned a message appears saying "We're sorry, something's not right", but does not say specifically what is wrong.

Did you confirm the bank account after adding it?

Is the name on your bank account the same as the name on your Paypal account?

 

Have you still got the direct debit mandate for Paypal showing in your direct debit list on your online banking?
