How do i get a PayPal account statement that has my name, address and account number?

Paypal accounts do not have numbers, your email address identifies your paypal account or your number if using the app.

I understand you're looking to get your PayPal account statement. As @kernowlass mentioned, the email address that you use to log in is the main identifier of your account. Your PayPal account doesn't have an account number. Here's how to view and download statements and reports of your PayPal transaction activity:

Go to Settings.
Click Statements and Taxes.
Click All transactions.
Click custom statement. 
Select the transaction type, date range, and file format.
Click Create Report.
When the report is ready, click Download.
