200 Transactions (IRS) Question

I understand that you must reach 200 payments (& also 20,000 gross) to be reported with a form to the IRS.
My question is-
What if you offered combined shipping for a buyer who purchased multiple items. Although you only receive ONE payment (the payment is made up of multiple purchases, so would you include all of the purchases as seperate payments) or are they only counting the amount of payments that you see on the transaction history page (from Jan 1, 2011 - Jan 1 2012).
