Upload file to support claim in Resolution Center

When I try to upload files to support my claim - click on Attach File and it then takes me to  a page saying "Contact Us - My Account Profile - Name Change"  we are unable to change name.....

I have tried several times, logged out and back in and I keep getting taken to this page.

 

an anyone tell me what the heck is going on?

I need to complete this.

I would recommend reaching out to our Customer Support via Twitter or Facebook, they're really great at assisting our customers in uploading documents and making sure your request is reviewed by the relevant team.
