Where do I upload the required documents to change my name?

Where do I upload the required documents to change my name?

I go to Account Settings - Account owner information - I try to change my name, but I get this message:

"You can make minor changes to your own name on this page. For other changes, you can send us documents that show your name. We can't transfer accounts to another name."

So WHERE do I upload or send these documents to? 

There is no option to upload anything.

Any help?
