The company logo doesn't appear when viewing or printing an invoice

For about a week now, when I view an invoice, the logo occasionally appears, but doesn't appear on the majority of invoices I print out. Is anyone else having this problem as this seems to be a new bug and I can't find anywhere to report it other than here?
