phone jack not recognizing the card reader

Every time I plug in the card reader it keep saying card connected then say card disconnected and the little head phone icon keeps flashing up and down

Try a different card reader to make sure the reader isn't defective.

Try the reader in a different phone or tablet to make sure it isn't the jack on the device causing the problem.

Make sure the jack is firmly inserted with no obstructions.

Re-install the app on the device.
