Is there a way to get two price drop down menus for one button?

I basically am going to be selling a specialty part that will be bought from people all over the world. I will be selling the part for $25 USD. Shipping is going to cost $4.90 to ship to any US address, $11.45 to ship to any address in Canada or Mexico, and $13.45 to ship to any other country not previously listed.
I want to have a single "Add To Cart" button that has two drop down menus; The top menu for quantity and the bottom menu for shipping. I have tried editing the code, but that has not worked. For an example of how I want to to look, please click here.
The way I have it set up there does not work as I want it to. If you select any of the three quantities, then select a shipping destination, then click "Add To Cart", the shipping destination cost will not appear.
I want to be able to accept one simple payment without having to send an email saying I need $XX USD more for shipping. I tried to add the shipping cost to the item cost, and it looks as though it would deter people from buying it.

Hi AA-airsoft!
Take out the second dropdown and instead, edit your shipping calculations in your Profile. The button will automatically calculate the shipping cost on its own based on where they're shipping it to!

Alright. I have done that, and it should be fine for now. However, I plan on selling more items soon, and I do not want to be shipping a box that weighs 10 pounds and is 3' x 1' x 5" for $4.90. I plan on shipping the small parts I made the previously mentioned button for via USPS Priority flat rate enevelope, and edited theshipping calculations so that no matter how many of those small parts they order, they will all be packaged into a single flat rate envelope and shipped for a certain price. So when I start selling larger items, wont the customers be able to choose those methods of shipping for the larger items? That is why I was wishing to make that special button with two drop-down menus.

It's the same process.
You're using a flat-rate envelope, so that should be one of the choices available in your shipping system if it's tied into the USPS rate quoting system.
It'll charge the same rate, based on the country involved, up to the limit of the flat rate envelope ( which is 4 lbs)

I have not set the weight of anything yet. For the shipping rates, I left it at:
From: 1
To: and up
Rate: $11.45
I am not too sure what the weight will be for each part yet. I am making them out of steel, and the part that the steel ones will be replacing is made out of a very light and maleable zinc alloy, so I cannot actually get an exact weight until I get a steel one.
But now I understand this whole thing much better, and will set the weight instead of amount ordered. If I have any other questions, I will be sure to ask. Thank you for your help Adrian and DealDude. I really appreciate it.
