Is it possible to implement this shipping scenario?

I'm working with a client with some unique shipping needs. We are using Paypal shopping cart and/or buy now buttons.
Is it going to be possible to implement this scenario throught he shipping profile and some button coding?
1. Shipping to continental US - shipping = 13% of total order
2. Shipping to other countries, US territories, AK and HI - shipping will be invoiced separately
So basically I need Paypal to charge 13 % for the continental US orders (which I know how to set up), and zero for everyone else, but somehow need to let them know that they will receive a separate invoice for custom shipping charge.
Any great ideas?
