How does a buyer who signed in under a guest profile confirm a item receit?

A buyer who made a payment as a guest can not confirm receipt. Because the buyer needs to login  on the account itself. You can advise the buyer to create a Paypal account under their name and email address prior on making a purchase.

With regards to your question, I'm afraid that the buyer who paid you under a guest profile does not have a way to manually confirm that they have received the item since it's a guest checkout.

If you are pertaining to your payment being on hold, once you have uploaded the tracking number, rest assured that your payment should be released one full day after the delivery date of your item.

How long does it take for a refund to go back to buyer?

The timeframes as to how long a refund would go back to the buyer differs on what payment method was used by the buyer on the transaction.
