Express checkout useraction=commit ignored, shows "Continue" button

Our Express checkout link sets useraction=commit in the query string, but live PayPal shows the "Continue" button and disclaimer rather than the "Pay Now" button. When the same code points to the Sandbox, from our staging server, it correctly shows "Pay Now".
Has anyone come across this issue before, and how did you fix it?
