My family has a t-shirt business and I built our online store using the "Add to Cart" buttons from paypal. It's worked great so far with one exception. We offer customizable options for our t-shirts when we are selling them at other venues, btu we have not been able to duplicate this option online, so we still find ourselves doing phone orders for these orders that have add-ons. Is it possible to set up "Add to Cart" buttons that start with a base price and then have dropdown menus to include add-ons to the basic item and to up the price depending on what and how many add-ons my customers choose?

Absolutely however, what you have in mind cannot be done using the online button creator. The online button creator was intended to generate basic item button code in order to get non-coders up and running quickly. Your scenario is outside the box and requires a small bit if custom scripting to manage the logic needed. The logic is done on your web or site, then the end results are uploaded to PayPal. Here's one example, here's another. And here's a different approach. And one more. It's all up to your coding imagination.
