All payments in the sandbox are under review?

This may be a silly question, but every "fake" payment I do in the sandbox comes up as under review? Why is that?
I am using paypal on a site that sells digital content. I don't ship anything. So when I do the fake payment process, I get the IPN notification, store the payment in a database and allow the user to download an item.
I can't get the item back, so how do I know that the payment is going to go through?
How would I check for that and why does Paypal have the most confusing API I've ever seen in my life?

Login to your Sandbox Developer Account.
Click on the Test Accounts link.
You should see your Test Accounts on the right side of the screen.
Notice the Payment Review column.
If you see Enabled for your Accounts, click on it,
You should now see Disabled.
