How is the 0.3USD transaction fee charged?

My question is,

Is the 0.3USD charged for every transaction you receive money from each buyer, or is it charged once for everytime you withdraw money from your PayPal account?

the 30 cents plus 2.9% fee is charged for every payment you receive.  there is no fee for transfering funds to your bank acccount in the USA.
