User requested a chargeback, then canceled it, and a few months later, it was completed anyway?

A user filed a dispute because he had not yet received the item.  He then got it a few days later and then canceled the chargeback.  A few months later, the chargeback was approved ANYWAY and I was charged with the price of the item + $20.  The user has already stated to me that he received the item and does not know why the chargeback was approved.  How can I get this rectified?
