form for return shipping refund will not submit

Attempting to file claim for refund of return shipping label costs. claim form will not submit [ it has no checkbox for EBAY RETURN SHIPPING LABEL CHARGES ] Live chat says "drag and drop " file to claim form, but ebay will not allow dragging and dropping from my account summary.

Now, I don't know what the form should look like but I strongly recommend contacting customer service at https://www.paypal.com/selfhelp/contact/call.
