Shipping vs. Pickup

I have what I thought would be easy, but turns out it is not.

I would like to sell an item with a "Buy Now" button and give the user the option of a flat rate shipping charge or in-store pickup.

I have tried this with/without shipping profiles & overriding them... 

Any suggestion on how to easily do this?
