Transfer went to my old bank instead of my current one.

Edit: Unhelpful community members will suggest that I simply changed it to the new account afterwards and forgot. I've already had this stupid argument with my significant other. I'm not interested in having it with strangers.
I while ago (well before 8/20/2022) I had Rivermark set up for withdrawals, but changed over to OnPoint Community CU. My Wallet now only shows OnPoint as an option for withdrawal.
But on 8/20/2022, transaction ID 5W841777KA6400507, I tried to transfer money to my bank, and the system tried to transfer it to my old account at Rivermark. It even has the last 4 digits of my Rivermark account number in the description.
A few questions:
Why is PayPal sending transfers to my old bank when my new bank is the only option I have set up now?
Why did it fail to deposit into my old bank, and get refunded back to PayPal (possibly because PayPal used Rivermark's Routing number, and OnPoint's account number for the transaction? If so...)
Why does the "transfer to" information show the last 4 digits of the Rivermark account number?
