Can't upload photo ID

Hello guys, i have problem when i tried to give the photo ID for verify account. I got notification "Your account access is temporarily limited" and in the bottom of notification i click provide information, then i move to this page

Then i click the resolve, and the form for upload photo ID showed. But when the form show, i can't click anything. Even the "Browse file", cancel button or the submit button can't be clicked. Anyone can help me to solve this problem?
