PayPal refunds

Does the recipient of a PayPal refund have to have a PayPal account in order to receive the refund ?  (ie- if the person paid with a credit card as a 'guest', will the refund automatically be applied to their card even if they don't have a PayPal account ?)

Yes, refunds go back to the funding source of the buyer.
