Error - The account you entered would need aditional review-- what?!

Okay, so to start out here, let me say that I've been a paypal user for years. I have long used it without any problems, and it's been great.

However-- I recently moved across the country, and had to set up a new bank account- thus I lost my bank account link with Paypal. Now I'm trying to set it up with my new bank account, and I'm getting nothing but grief.

This is a simple CIBC, Chequing account. There's nothing special about it.

The bank account i successfully linked to before was the same. (On the first try, too)

The first several times (yes, several) I've tried to get paypal to send the two small deposits to my bank account- nothing. Nada. I've kept trying (over the course of maybe three months now), and on trying again, I got this error message:

"The account you entered would need additional review. Please link a different bank account.

If you would still like to use this bank account, you may fax us a copy of the bank statement with the two amounts PayPal deposited in your bank account shown and a copy of your driver's license or other photo I.D. You must use *this form* for your fax cover sheet and send the fax to *Removed*"

First of all, I don't have another bank account. Aside from my savings accounts, and I'm not touching those. So that's not an option.

I haven't gotten two small deposits, I've been checking my bank account online. Now Paypal is telling me to fax them (I don't have a flipping fax machine!) with a copy of the bank statement (I just GOT here, I haven't even set up snail-mail statements yet!!) that shows the two small deposits they never sent. 

I've been entering my account number as such: XXXXX-XX-XXXXX (without the dashes, but you get the idea) - is this correct for a CIBC bank account? Do I leave out the first part? It's my new bank's institution number, but it shows up as part of my account number. Although I think I've tried it without that part too...

HELP! I'm about ready to start tearing out hair here!
