If you managed to imput a 4 digit code incorrectly x3 times then you would be blocked from using that system.
So to add your bank account you would need to contact customer services and as you can't use the code system they may require you to upload a copy of your bank statement to verify ownership of that bank account.
Click on the words "Help / Contact" at the bottom of paypals pages and use the phone option in the blue band at the top OR very bottom of page dependent on what country you are in. Scroll down past the list of help questions and click on 'Call us' bottom of that page. Log in and get the code.
