Donate and Subscribe Button in one form

I am writing a Donation page for a charity and would like to have both a subscribe button and a donate button at the end of a form.  I want all information filled out in the form can be sent with either option.  Is this possible?

http://www.restorationgateway.dreamhosters.com/give.php The page has several donation options, so there are several simple and short forms on the page.

I have been trying to find the answer in the Paypal help forum, but the links are either broken or point to another forum that just points back to paypal.

There may be some useful information about creating PayPal buttons within a form in the PayPal Developer Forum. That is where discussions about customized solutions take place, and the people there are in the best position to offer you suggestions.
