An unauthorised transaction means that an account holders Paypal account was hacked and used without their consent by your ?buyer.
If Paypal check IP addresses etc and find in favour of the account holder then of course they are reimbursed.
However you would not lose out either as long as you met ALL the requirements of seller protection, so your best bet is to go and read up on it to make sure you were covered.
An unauthorised dispute does not need to be escalated to a claim, paypal investigate the minute its been reported.

As i said if it was an unauthorised payment ie someones paypal account was hacked then of course they are reimbursed.
If you were not reimbursed under paypal seller protection then you could not have qualified for that protection, best read up on it to see why you missed out.
The most common reason is by selling virtual / digital items, you can sell them but as there is no seller protection its at your own risk.
