Custom PayPal Selection for Donations

First of all, I would like to thank anyone who would be able to help me. 

If you go to http://www.hcri.com

And click on the Donate Button

you will see two Donate Buttons 

1 for Donate to HCRI and 1 for Joshua's House

And when you click on either button you wil see this come up.

What we have now. We can not see that you have choosen Donate for Joshua's House. It just shows The Health Communication Research Institute, Inc. 501c3

This is what it shows now. And this is not what we want it to look like

What we want is that when you click on the Donate for HCRI we would like that the  the Donate Page would show like what we have on the Graphic below.

 

And when you click on Joshua's House we would like it to show Donate to Joshua's House, like on the graphic below.

 

How can I get the Donate Page look like what I have discribed?
