Duplicate orders

I've been using PayPal for a number of months and in about the last two months, every order placed through PayPal gets placed twice, but only charged once.  This creates confusion for my customers.  I use ZenCart for my ordering software and the one thing I'm seeing is when this happens one of the duplicate orders says "web_accept" and the other says "unique."

 

How do I fix this so that a customer's single order only goes through once?
