I received a request of "proof of address" to remove the limitation in my account, and I'm fighting with it for almost three weeks because none of the official documents I'm offering is accepted. Below there is the list of documents that are accepted as a proof of address, followed by my answer:
What is a valid proof of address?
We accept a copy of:
- Utility bills (phone and broadband services, health insurance, gas, water, electricity, etc.)

Yes, I have it under my name, but I'm not receiving any statement anymore because they are releasing everything online. I can print bank statements but my first name is not written in full but only with the initial followed by my surname and it is followed by the IBAN and not by my address. Then I sent to you my "personal information" of my bank account, in which my name is written in full and also my address is shown, but it was not accepted.
- Your driving licence. (Note: We can only accept a driving licence if: it shows your address, it's been issued within the last 12 months, and it's accompanied by another valid government-issued photo ID)

Therefore, as I proof of address I sent my ID, in which there is also written my address and my name in full, but it was not accepted because there is not written the zip code and because it was released last year and I could have changed my address.
Then, as a last option, I went to the municipality of the town where I'm living and I ask for an official document attesting my residence, specifically asking to write my name in full, since in every document released by the country I'm resident, only the initial of the first name, followed by the surname, is written. They released me without any problem the document requested, showing my name in full, my address including the zip code, my birth date, my birth country, and also the name of my parents, it was released three days ago, it is an official document of the municipality signed by the Mayor, but, with my big surprise, also this one was not accepted.
Honestly I'm shocked. Why an official document released by the municipality and signed by the Mayor, that is used to rent/buy a house and to open a bank account, is not accepted?
What else can I show you more than what I already sent you? You have my official ID, you have all the details of my bank account and bank credit card, you have the contract of my health insurance, you have a document released by the municipality, what else do you need? I finished the list of available documents......
As I don't have a job, I'm doing occasional jobs, I gave you everything you asked me and you don't accept any official document, what else can I do to get back my own money and my account?

You need to ask customer services what they will accept, no one here can help you .
You are also posting on the UK forum and I don't think you are in the UK as we don't pay for healthcare here its all free.
