Special Instructions field doesn't show up for my customers.

I set up four buttons each with the option for "Can a customer add special instructions in a message to you?" set to "Yes" under Step 3, but none of my customers can find the field where they enter special instructions when they go to purchase either via a PayPal account or as a guest using a credit card.  I am currently using a "Pay Now" button for each button.

I'm sorry to hear about the trouble with the buttons you've created - that definitely sounds frustrating. In this case, I would advise checking our Merchant Integration Technical Support Community for more information and to see if anyone else has had this issue as well.
