USPS scan form 5630

Does PayPal use it?  I've been wondering

I know that this is an older post, but in case you haven't found your answer, the "Scan Form" which is available to print, is the form 5630.

I am having an issue printing it, as it prints about the size of a postage stamp, and is entirely unuseable, but I'm assuming that it works for others.
