Have you withdrawn funds before?

Which funding source are you withdrawing to bank account or debit card?

Have you confirmed both funding sources?

Does the name and address of the bank and card accounts exactly match your paypal account name?

Are the funds cleared and not pending?

Are the funds in USD?
