Need to Prevent non-US shipping and non-US PayPal - Account Settings still allow non-US transactions

I need to prevent non US shipping and PayPal accounts. Already checked my settings, but transactions still complete with a Non-US shipping address.  I just had to refund a customer purchase from Canada.  Thank you!

Maybe also enabling the option that blocks unconfirmed shipping addresses will help reduce if not eliminate this kind of situation:

Otherwise, there is no such blocking preference with shipping addresses in PayPal settings.
