I also struggled for some time as some advice had half right however got it added as follows:

Bank - first 6 digits which your bank shows as xx-xxxx (DO NOT USE DASH)

Account number - (9 or 11 digits) xxxxxxxx-xx (EVEN IF BANK HAS 3 DIGITS for suffix - use ONLY 2) and YOU  MUST PUT THE DASH

So - Bank NO DASH; ACC number WITH DASH
