I received a payment of $800 from my client in USA, from which I received $758 that is currently in hold as it's my first transaction. Now if the sender wants a refund of money, will the sender get full refund of $800? Basically is the full amount refunded to him or only $758? I don't have to pay any additional fees from my account so that he gets full amount back?

If you issue a full refund then he gets a full refund.
If you don't have the 'full' amount in your balance then the 'fee' element would be deducted from your linked bank account.
