Hi, need advice please, someone has opened a case against me in PayPal for something bought over 2 months ago, I'm the seller by the way."....if PayPal find in buyers favour, do they always insist that buyer sends item back first at their cost, before refunding them. Terry

Ask for the item to be returned within 7 days(if thats feasible) in the original condition sent and a full refund will be issued

If u are a private seller u dont have to pay for return posage unless u wish to

Do not refund unless u receive the item back and are happy with it

Only reply on the dispute not thru private emails

If u havent received the item within 7 days come back for more help

In answer to your question, no they don't ALWAYS tell the buyer to return the item first BUT it is the norm as long as the item is not counterfeit for eg. 

I would not offer for the items to be returned as the buyer has had them for soooo long and paypal MAY find in your favour.

All you can do is await paypals decision.
