My account have been  permanently limited due to some  potential risk as per the paypal guideline . is there any one who can tell me that what should i do now ? what type of documents should i submit to paypal ? where should i send a message to paypal support ?

Sadly permanent means permanent, you have been banned from using paypal.

If you could provide documents to resolve it then paypal would have made it temporary.
