When Paypal tallies up my yearly "sales" to determine if a 1099 is required, do they include every single received payment even if they were not sales, but were friends or family sending me money through my PayPal account?

Friends and family payments are not included. What you receive in gross for sales if over $20K and over 200 transactions you'll receive 1099 except for VT and MA, $600.
