Unable to activate Business Debit Card

I've done my own research and have been provided with the following information:

Unfortunately, I am unable to locate the a paper bank funding instruction or the address to which is to be sent. Can someone help me out. Thanks.

When you know the 4-digit code:

1. Go to money.
2. Click Unconfirmed next to the bank account you want to confirm.
3. Submit the required information.  (You can make 3 attempts.)
   1. If the bank account is for a business or more than one person is required to authorise debits, you must print and sign a paper Bank Funding Instruction and post it to the address provided on-screen.

Unfortunately, I am unable to ;plate the a paper bank funding instruction or the address to which is to be sent. Can someone help me out. Thanks.
