how can I add a Text Field to the new Smart Buttons?

I want to add a Text Field to the new PayPal Smart Button (i.e., to ask buyer for special instructions).  You could do this with the older PayPal buttons, but I don't see the option to add a Text Field with the new Smart Button.

Basically, you get the value of your text box and pass it to the custom_id or description fields of the button's createOrder function.
