problem whenever a payment is made over £45 inserting card

driving me mad. if a customer spends over £45 it is hit and miss as to whether or not the machine will work when someone puts their card in the card reader. when it isn't working, it will say 'pin ok' on the actual card reader BUT my phone says DECLINED (which is genuine, the payment has not gone through which is really annoying especially as i have to try and explain to a buyer that their payment hasn't gone through although they do accept this when i show the the large in red 'declined' on my phone but the 'pin ok' doesn't exactly help which is what it says on the reader).
any ideas as to why more often than not when someone attempts to make a payment putting their card in the machine as opposed to tap payments, it won't work
