Refunds Failing

I have an unusual problem and wondered whether it is an eBay or PayPal problem.

I have an open case and need to refund a buyer, plus I have an open case where a seller is refunding me. Both our refunds have failed. So I have issued a refund to a customer, but it failed and a seller issued a refund for me and that also failed?

I am at a loss as to what the issue is here. The buyer in question is particularly nasty and I was very keen to refund ASAP, now she will negative me and I will probably receive more abuse from her over this.

I am desperate to get these two cases sorted so I can take a nice long break from eBay.

I will be very grateful for any advice anyone can give me regarding this issue of refunds failing.
