I sold something they sent it back do I still get charged the PayPal fee

When a buyer pays you are normally charged 3.4% + 20p.

If you have to issue a refund (make sure you use the refund option) then the 3.4% is refunded at the same time you make the refund but paypal keep the  20p as a service charge for processing the original payment and the refund for you.
