They really don't; instead you do by creating a subscription/installment/automatic billing button (or email urls) or have PayPal checkout integration that sets up automatic payments or set up a billing agreement and they just checkout and in the process PayPal sets up the billing agreement in the background and in to buyer's PayPal account profile while asking buyer to add a back up payment method.
