Can I use the custom credit card/Debit card form to collect payment on my website? Is it possible?

If you want to process a credit card directly you will need to sign up for a Braintree account. PayPal Pro would work, too, but it has a monthly fee and Braintree does not. Braintree is PayPal owned, of course.
The Braintree drop-in UI would actually replace most of the fields you see here. It would include credit card, venmo, PayPal, and even Google/Apple Pay if you have it enabled.
