How do i calculate my minium payment.

I'm trying to calculate what my new minimum payment will be but the explanation doesn't make much sense I have    got 4 separate items which i have until August to pay off, Do I just add those amounts together and / 4?

 

Or do i take the entire balance and /12 ?
