So i sold a watch for $800 and paypal took a fee of $ 23.50, so technically I only receive $776.50. So if the buyer want a refund and paypal refund the buyer the amount of $800 , do I have to pay an extra of 23.50 to paypal??

Your paypal fee is 2.9% + 30 cents per transaction.
If you use the REFUND option then put in the full amount paypal will refund the 2.9% as part of that full payment but you lose the 30 cents as a processing fee.
Advice is voluntary.
