I made a sale on eBay to a US customer (I am also in the US) for $220. I expected the PayPal fee to be $6.68 ($0.30 + $6.38 (2.9% of 220)). My total PayPal fee was $9.98. Can anyone explain why the fee would be 50% more than expected?

They may have a U.S Paypal account but they may have been abroad when they paid you so international / cross border fees would have applied.
