Help, BPI Bank Account got blocked

I linked my BPI bank account last week and it got blocked 
how do i unblock or what is the reason why it is blocked?

Do you have the correct bank routing code – also called 9-digit bank code (please be advised that this is not the card number nor the account number)?

Registered name and mailing address with Paypal must match with the one on the bank account?
