4 digit code missing

it's almost a month since i requested for a confirmation code to link my card to my account and my fund source already charge of the code generating cost of 2.057$ three times 'coz i clicked the request button several times with a notification that the amount will be credited back to my fund source if the code is not received, can somebody help me by telling me what to do? thanks and God bless.
