How to integrate paypal shopping cart with custom forms

I currently have a website on which I sell custom printed products. Each item requires personal information to be entered by the client when placing their order. How can I create an "add to cart" function that will also allow clients to input their information into the form fields and be included with their order? I need to integrate my forms with an add to cart button, but it once the screen moves on to collect payment information, it cancels out the other portion of my form which is very important, and must be posted to my email account. Other printing sites have this option, how can this work?

I do know it is possible to create a custom form however; this is not my area of expertise. I would advise you to hop on over to the PayPal Developer Forum where they would be able to better assist you with your specific form, here is the link.
