It's not possible to customize or modify the email template that is sent from PayPal to a merchants customers.
If you want to send a custom confirmation email/receipt for an order, it will need to be developed and programmed into your system. PayPal's Instant Payment Notification can be setup to send messages about transactions to your website, and from there—to your customers.
